If you are a new patient, please complete the appropriate patient forms before your appointment. Filling out your paperwork ahead of time gives our team the information we need to better understand your health history, current symptoms, previous injuries, and the reason for your visit.

Auto Accident Patient Forms
If you are visiting Lake City Chiropractic following an auto accident, additional information about the collision and your injuries may be needed.
Please complete the appropriate auto accident paperwork and provide as much information as possible about the accident, your symptoms, and any care you have already received.
What Should I Bring to My
First Appointment?
In addition to completing your patient forms, please bring any information that may be relevant to your visit.
This may include:
- Photo identification
- Insurance card, if applicable
- A list of current medications
- Relevant medical records
- Recent X-rays or imaging, if available
- Information related to an auto accident or injury claim, if applicable
